Flat Feet in Children - Expert Orthotic Treatment in Delhi & Gurgaon

Flat Feet in Children Treatment.webp

Introduction

Flat feet in children are one of the most common concerns parents bring to us — and one of the most misunderstood. Many children have flat feet that are completely normal and require no treatment. But some children have flat feet that cause pain, affect walking, and limit participation in sport and play. Knowing the difference is the key. At The Rehab Street, our paediatric orthotists carry out a thorough biomechanical assessment to determine whether your child's flat feet need support — and design custom orthotics that fit the way they actually move.

What Are Flat Feet in Children?

Flat feet (also called pes planus or fallen arches) occur when the arch of the foot is lower than normal or absent entirely. In children under the age of 3–4 years, flat feet are developmentally normal — the arch is covered by a fat pad and develops gradually as the child walks. Most children develop a visible arch by age 6–8 years. When the arch fails to develop, or when flat feet cause symptoms, specialist assessment is recommended.

Types of Flat Feet in Children

Flexible flatfoot is by far the most common type, accounting for over 90% of cases seen in paediatric orthotics clinics. It does not always require treatment — but when it causes pain, fatigue, or gait problems, custom orthotics make a significant difference.

Normal vs. Cause for Concern — When Should You Act?

Use this guide to understand when flat feet are a normal part of development and when to seek an expert opinion:

If you are unsure, a single assessment appointment is all it takes to get a definitive answer. Our orthotists will tell you clearly whether your child needs support or simply needs time.

How We Assess Flat Feet at The Rehab Street

Our flat feet assessment is child-friendly and typically takes 45–60 minutes. No GP referral is required.

Step 1 — Developmental History

We ask about your child's walking history, activity levels, family foot history, and any current complaints of pain or fatigue.

Step 2 — Foot Posture Assessment

We use the Foot Posture Index (FPI-6) — a validated clinical tool — to score your child's foot alignment in standing, sitting and walking.

Step 3 — Gait Analysis

We observe your child walking and running, looking for overpronation (inward rolling of the ankle), toe-out walking, and asymmetric loading between feet.

Step 4 — Plantar Pressure Mapping

Digital pressure analysis shows us exactly how weight is distributed across the foot — often revealing overloaded areas in the arch and midfoot that are invisible to the naked eye.

Step 5 — Flexibility & Strength Testing

We test ankle dorsiflexion (calf tightness), subtalar range of motion, and arch flexibility — distinguishing flexible from rigid flatfoot and ruling out underlying neurological or structural causes.

Step 6 — Written Report & Treatment Plan

You receive a plain-language written report with our findings and a personalised treatment roadmap.

Flat Feet Treatment Options for Children

Not all flat feet need treatment. When treatment is recommended, it is always evidence-based and specific to your child's presentation:

1. Custom Paediatric Foot Orthotics

The most effective and most commonly recommended intervention. A custom orthotic is moulded to your child's specific foot shape and designed to support the arch, control overpronation, and redistribute pressure away from painful areas. Unlike generic arch support insoles from a pharmacy, a custom orthotic is biomechanically prescribed — it works with your child's movement pattern, not against it. Most children notice improvement within 3–6 weeks of consistent use.

2. Footwear Guidance

The wrong shoes can make flat feet significantly worse. We advise on specific features to look for: a firm heel counter, a semi-rigid midsole, and adequate toe box width. We can also advise on brands and styles suitable for the Indian market that work well with custom orthotics.

3. Stretching & Strengthening Programme

A targeted home programme focusing on the calf muscles, tibialis posterior (arch muscle), and intrinsic foot muscles. Takes under 10 minutes per day — designed to be simple enough for children to do independently from age 5 upwards.

4. Activity & Sport Recommendations

We advise on which sports and activities help develop arch strength naturally — swimming, cycling, and barefoot walking on uneven surfaces all have documented benefits for paediatric flat foot. We also advise on surfaces and footwear for specific sports.

5. Review & Growth Monitoring

Children's feet change rapidly. We schedule 6-monthly review appointments to reassess the arch, update orthotic prescriptions as the foot grows, and track progress. Orthotics are typically replaced every 12–18 months in growing children.

Frequently Asked Questions (FAQ)

Q1. My child is 2 years old and has flat feet — is this normal?

Almost certainly yes. Flat feet are completely normal in children under 3 years. The arch develops gradually between ages 3 and 8. If your child is walking normally, showing no pain, and their flat feet appear on both sides equally, there is typically no cause for concern. A reassessment at age 4–5 is a sensible milestone check.

Q2. Will my child grow out of flat feet?

Many children do — particularly those with flexible flatfoot. Studies suggest that approximately 50% of children with flat feet at age 5 develop a normal arch by age 10 without any treatment. However, children with rigid flatfoot, pain, or associated gait problems are unlikely to self-correct and benefit from early orthotic support.

Q3. Do orthotics prevent the arch from developing naturally?

No — this is a common misconception. Orthotics do not weaken the foot or prevent natural arch development. The evidence shows that custom orthotics prescribed for symptomatic flat feet improve pain and function without interfering with normal development. We always include a strengthening programme alongside orthotic use.

Q4. My child complains of leg pain at night — could it be related to flat feet?

"Growing pains" — cramping or aching in the calves and thighs at night — are sometimes linked to overpronation from flat feet. The compensatory muscular effort of walking with a flat foot all day can cause cumulative fatigue that presents as night pain. An assessment will tell us whether orthotic support would help.

Q5. How long will my child need to wear orthotics?

This depends on the type of flatfoot and how the arch develops. Many children wear orthotics from ages 4–10 and can be weaned off as the arch matures and strengthening exercises take effect. Children with rigid flatfoot or underlying structural causes may need long-term orthotic support. We reassess at every follow-up appointment.

Q6. Can flat feet affect posture and the spine?

Yes, over time. Overpronation from flat feet causes the lower leg to rotate inward — which tilts the pelvis and can increase lumbar spine stress. Children with untreated painful flat feet sometimes develop knee pain, hip pain, and lower back pain as secondary problems. Early orthotic support prevents this chain of compensatory effects.

Q7. My child is in school sports — will orthotics fit in their sport shoes?

Yes. We design orthotics with your child's primary footwear in mind, including school shoes, sports shoes and cleats. Bring the shoes your child wears most to the appointment, and we will profile the orthotic to fit correctly in each pair.
