About the Paediatric Orthotics Clinic

The Paediatric Orthotics Clinic at The Rehab Street provides specialist assessment and customised orthotic solutions for children with conditions affecting their muscles, bones, joints, posture, alignment and movement.

Our clinical orthotists work closely with parents and caregivers throughout the assessment, design, fitting and follow-up process to provide patient-centred care for every child.

Children with conditions such as clubfoot, cerebral palsy, flat feet, toe walking, ankle or foot deformities, gait abnormalities and other musculoskeletal conditions may benefit from an orthotic assessment, depending on their clinical needs.

Both clinic locations — in Greater Kailash-2, Delhi and Sector 50, Gurugram — offer paediatric orthotics consultations with no referral required.

When Should You Consult a Paediatric Orthotist?

Parents may consider a child gait assessment or orthotic consultation if their child has:

  • Unusual foot or ankle alignment — including feet that turn inward (intoeing) or outward (outtoeing)

  • Flat feet associated with pain, fatigue or difficulty with physical activities

  • Frequent tripping, falls or difficulty maintaining balance during walking or running

  • Toe walking or an unusual, asymmetrical walking pattern

  • Difficulty standing, walking or participating in sport and physical activities

  • Differences in leg alignment, posture or movement — including one leg shorter or more pronated than the other

  • A diagnosed condition such as cerebral palsy, spina bifida or clubfoot for which customised orthotic support has been recommended

These signs do not automatically mean that a child needs orthoses. A clinical assessment with our paediatric orthotist will determine whether orthotic intervention is appropriate.

What to Expect at Your Child's Appointment

1. Initial Clinical Assessment

During the first appointment at our Delhi or Gurugram paediatric orthotics clinic, the clinical orthotist will discuss your child's concerns, medical history, developmental milestones and daily activities with you.

The assessment may include:

  • Observation of standing posture and lower-limb alignment

  • Assessment of foot and ankle position

  • Evaluation of joint range of motion and muscle strength

  • Observation of walking pattern and gait — including a full child gait assessment where appropriate

  • Assessment of balance, stability and functional mobility

The assessment is tailored to your child's age, diagnosis, symptoms and individual needs.

2. Individualised Orthotic Plan

Based on the clinical findings, the orthotist will explain the available treatment options and determine whether a customised orthosis — such as an AFO, SMO, UCBL or custom insole — may be appropriate.

The plan considers your child's functional goals, comfort, activity level and specific clinical requirements.

3. Measurements and Casting

If a custom-made orthosis is recommended, the clinical orthotist will take the necessary measurements and, where required, a cast or mould of the relevant body part.

These measurements ensure that the orthosis is designed precisely to your child's individual anatomy and clinical requirements.

4. Custom Orthosis Design and Fabrication

The customised orthosis is designed and fabricated according to the assessment findings and the child's specific needs — aiming to provide appropriate support, alignment and function while prioritising comfort and daily usability.

Fabrication time varies depending on the type and complexity of the orthosis prescribed.

5. Fitting and Adjustments

At the fitting appointment, the clinical orthotist will assess the fit, comfort and function of the customised orthosis on your child.

Parents and caregivers will receive clear guidance on the correct application, removal, wear schedule and care of the device. Adjustments may be made at any stage when clinically required.

6. Follow-Up Appointments

Follow-up visits allow the clinical orthotist to review your child's progress, assess the orthosis fit and function, and make any necessary adjustments.

As children grow and their clinical needs change, periodic reassessment ensures that the orthosis continues to provide the right level of support.

The appropriate orthosis is selected by the clinical orthotist after assessing the child's condition, alignment, muscle function, gait and functional requirements. Not every child requires an orthosis.

Frequently Asked Questions

Do I need a referral to book a paediatric orthotics assessment?

No. You can book directly at either of our clinics — in Greater Kailash-2, Delhi or Sector 50, Gurugram — without a referral from a doctor or physiotherapist.

From what age can children be assessed?

We assess children from infancy through to adolescence. The assessment approach is adapted to the child's age, developmental stage and clinical presentation.

How do I know if my child needs orthotics?

Common signs include frequent trips or falls, complaints of foot or leg pain, flat feet, toe walking, turned-in or turned-out feet, and difficulty keeping up with peers during physical activity. A clinical assessment with our paediatric orthotist will provide a clear answer.

Will the orthotics fit inside normal school shoes?

In many cases, yes. Custom insoles and SMOs are designed to fit standard footwear. For devices such as AFOs, we may recommend specific shoe styles and can advise you at the fitting appointment.

How long does fabrication of a custom orthosis take?

Fabrication time varies by device type and complexity. We will give you a clear timeframe at the assessment appointment. Most customised insoles and SMOs are ready within 2–3 weeks.

Can orthotics help with cerebral palsy or other neurological conditions?

Yes. Ankle-foot orthoses (AFOs), KAFOs and HKAFOs are commonly prescribed for children with cerebral palsy, spina bifida and other neurological or musculoskeletal conditions. The appropriate device is determined following a full clinical assessment.

How often will my child need to come back for follow-up?

Follow-up frequency depends on the child's age, rate of growth and clinical needs. As a general guide, we recommend a review every 6–12 months, or sooner if the orthosis no longer fits comfortably or the child's needs change.
